Evaluative arthroscopy

It is not justifiable to use it as a diagnostic tool or for joint lavage in osteoarthritis as it is invasive and expensive. It is used mainly to cure mechanical symptoms in osteoarthritis like locking from a torn meniscus, to remove a loose body or trim a chondral flap. It is used as an adjunct to high tibial osteotomy or before mosaicplasty. In ligament injuries, the diagnosis is usually obvious from a clinical examination. An examination under anesthesia and knee arthroscopy confirms the clinical diagnosis. The preliminary scopy allows the surgeon to probe the menisci and survey the rest of the knee. Next it allows the performance of a menisectomy and notch plasty if needed.The ACL reconstruction then follows by the arthroscopic assisted technique. In the case of young individuals with unexplained knee pain and nearly normal X rays, the arthroscope may pick up silent chondral lesions which could be an enigma otherwise. Meniscal tears, cartilage tears and loose bodies are the main indications. It can be used to perform cartilage surgical procedures for early cartilage lesions.It is definitely not useful in chronic cases of knee pain where the ortho surgeon can clearly see evidence of Joint space narrowing on X rays.
